Where is your office located?
My office is conveniently located just off the Edens Expressway at the intersection of Central Avenue and Crawford Avenue, less than five minutes from Old Orchard shopping center. It’s close to public transportation for those in the city. And there is plenty of free parking, and a comfortable Starbucks next door for pre- and post-session enjoyment.

Yes, about half of my clients utilize their insurance benefits. I am an in-network provider for Blue Cross/Blue Shield PPO, Humana/Lifesync/Trustmark, and Cigna insurance and EAP, and I am happy to bill these companies directly for services. In most cases, after your deductible is met, you will be responsible only for a small co-pay for each session.
I am an out-of-network provider for most other insurance companies. In these cases, I ask that you pay for your therapy at the time of service, and I can then provide you with the statement necessary for you to submit to your insurance. Depending on your policy, you may be able to obtain reimbursement at a rate close to your in-network benefits.

What is my outpatient mental health coverage?
What is my deductible and has it been met?
What is my coverage or copay amount per session?
How many sessions are covered in a calendar year?
What is my coverage for an out-of-network provider?

Please download, if possible, and complete these forms, and bring them with you to the first session.
Also, verify your benefits with your insurance company and please bring payment for your session, deductible or copay, if applicable. Finally, clarify for yourself where you’d like to start and what you initially want to accomplish in therapy. You may jot some things down to help you remember. Of course, you do not have to have everything figured out and some of these goals may change through our work together. Don’t worry if it’s hard to know where to start; that’s something we can also figure out together.
You can make your payment by check or cash, and I do take credit cards: MasterCard, Visa, and Discover. You can also use your Flexible Spending Debit Card, which is typically through MasterCard or Visa.
